With over 150,000 pages of medical records and 7,000 active customer files, their operations were highly manual, inefficient, and prone to errors. Data was stored in disconnected systems, creating silos that hindered coordination.

We digitized over 150,000 pages of medical records and centralized them into a single, easily accessible database. This back-end system was designed to integrate seamlessly with external health sources, facilitating real-time data exchange. Additionally, we developed a foundation for future enhancements, such as integrating address-based bus stop identification.

Front-end Solution:

We created an intuitive online portal and mobile app that simplified the application process for customers, allowing them to apply digitally or via paper forms, ensuring accessibility for all.
For paper applications, we implemented an AI-powered scanning system that digitizes forms, automatically extracting key data fields and entering them into the same centralized system as digital applications. When the AI is under 70% accurate on certain fields, it flags them for human review, ensuring accuracy and reducing potential errors.
This hybrid approach of AI automation and human oversight streamlined the process, minimized errors, and provided real-time status updates, enhancing transparency and customer satisfaction.

How They Connect:
The back-end database and front-end applications were integrated through Power Automate flows and a robust API, enabling real-time data synchronization. This connectivity ensured that the occupational therapists and doctors would pull accurate data from the centralized system while pushing updates seamlessly. This eliminated data silos and provided a unified view of the program’s operations.
